Send one clear email

Do not use Placiva for emergencies, urgent medical decisions, or urgent home safety repairs.
Use the email address connected to your request when possible. Say whether you want access, correction, deletion, or opt-out help.
Include the page URL, the exact claim, and a better source if you have one.
Send the provider name, page URL, service area, and what appears mislabeled or outdated.
Describe the page, the control or content that caused trouble, and the browser or assistive technology if you know it.
Use the provider form first when you can. Email if you need help understanding labels, scope, or pilot terms.
For emergencies, urgent repairs, medical decisions, or immediate home hazards, contact local emergency services or a qualified local professional.
Do not send private documents
A first email should not include medical records, financial documents, Social Security numbers, passwords, faces, or private family documents. If a provider later needs sensitive information, confirm why and send it through a channel you trust.
What makes a request easier to handle
- Use a clear subject line, such as Privacy request, Source correction, Provider label issue, Accessibility issue, or Sponsor question.
- Include the URL when your message is about a page.
- Use the same email address you used in a form if you are asking about your own submitted request.
- Tell Placiva if the issue is privacy-sensitive or accessibility-sensitive.
